Bromley LINk is trying to pull together people’s experiences to find out what’s working well, and not so well, with the discharge procedures in Bromley. If you can help, download their questionnaire, complete it and send it back (it’s free!)
This is a message from LINk.
As you will hopefully be aware, for some time Bromley LINk has had an interest in hospital discharge (the process of leaving hospital). Several members have told us that they, or someone they know, has experienced problems with the discharge process and so the LINk Coordinating Group has decided to develop a questionnaire to help us learn more about what is and isn’t working well in hospital discharge in Bromley.
The questionnaire is here, and all LINk members will be getting a hard copy. If you have been in hospital in the last 6 months, please do fill it in to let us know about your experience – whether it was good or bad! If you know of anyone else who has been in hospital recently, please do encourage them to complete the questionnaire too – either download it from here, or contact the LINk office and we will send one out. The more responses we receive, the greater the weight that our results will have!
The questionnaire should not take more than 10 minutes to fill out – on the 18th March we will conduct a draw of all named responses that we have received, and one lucky winner will receive £50 of Marks and Spencer vouchers. Participation in the survey is voluntary, your answers are completely confidential, and will not be linked to your medical records.
The LINk’s Coordinating Group has also commissioned an outside agency to do some in-depth patient interviews about experiences of the discharge process so if you would be happy to talk to one of the researchers about your experience in more detail, please tick the box at the end of the form.
We will pull together all the information we receive into a report that we will send to local NHS organisations and the Care Quality Commission, so that we can try and improve hospital discharge for everyone in the borough.
